How Our Health Coaches Make an Impact:

Health Coaches are at the center of Diverge’s care model. Across all markets, you’ll support patients with chronic conditions like diabetes, asthma, COPD, CHF, and hypertension—often by meeting them in their homes or community settings. You’ll provide patients with one-on-one support to help build healthier habits, overcome barriers, and stay engaged in their care. You’ll partner closely with Nurse Practitioners, Social Workers, and Health Coach Managers to coordinate care, address medical and social needs, and ensure patients feel supported. This role is ideal for someone who is committed to learning, mission-driven, and passionate about helping others. You’ll thrive if you love learning, adapt quickly, and enjoy building trusted relationships in the communities we serve.

What is a Health Coach?

A Health Coach supports patients one-on-one to build healthier habits, manage chronic conditions, and overcome barriers that affect their well-being. They meet patients where they are—at home, in the community, or virtually—to provide guidance, motivation, and resources that make everyday health easier to navigate.
